Your appearance is very important in an interview. You must take all possible precautions to ensure that you project a neat, clean, and professional image to your prospective employer.

What you wear...
Dark colors work best at portraying a professional image. Wear a dark-colored suit (black or navy is preferable) with a white shirt. Be sure that your clothing is clean and neatly pressed. Men should wear a conservative tie. Women should wear nylons if legs are exposed.

Shoes...
Be sure that your shoes are clean and free from dirt, mud, and scuffs. They should be comfortable and easy to walk in, as you never know when you may need to climb stairs or take a building tour.  Never wear flip flop shoes and if opened toed shoes are your preference be selective when choosing them, and if you do make sure your toe nails are neat.
Hair...
Be sure that your hair is clean, neat, and out of your eyes.  If they can't see your eyes they might think your trying to hide something because good eye contact is key.  
Miscellaneous...
Make sure that fingernails are clean underneath. For the ladies...if you are wearing nail polish, be sure that it is free from chips, etc. Jewelry, perfume, cologne, and make-up should be kept simple.
Accessories...
When possible, your shoes, belt, and purse should match. All should be clean and in good repair. Your folder or briefcase should contain a copy of your resume, your reference sheet, and a pen or pencil. Remember that your goal is to look professional.
Smile...
This is perhaps the single-most important thing you can wear to an interview. No outfit is complete without one.
Do a double take...
Before going into the interview, stop by a mirror (preferably in a restroom) and double-check everything mentioned above. Practice smiling. Be sure there is nothing in your teeth (food) or on them (lipstick). Put your shoulders back, and knock 'em dead.
